Anadrol (Oxymetholone): Strength and Mass

Anadrol (oxymetholone) is one of the most potent oral mass-building steroids, and also one of the harshest. The short answer: it is a 17-alpha-alkylated DHT derivative with an 8-hour half-life, severe hepatotoxicity, and strong erythropoiesis. It produces rapid weight gain, much of it water, and carries a heavier side-effect burden than almost any other oral in this dataset. This is a compound where liver enzymes, blood pressure, and hematocrit monitoring are the entire harm-reduction story.

What Anadrol is and why it is unusual

Anadrol is a 17-alpha-alkylated DHT derivative. It was originally introduced clinically for anemia and osteoporosis, and is still used for hereditary angioedema. The compound binds the androgen receptor and drives strong erythropoiesis (red blood cell production), which is the basis for both its anemia use and its mass-building reputation.

Here is the unusual part. Despite being a DHT derivative, oxymetholone aromatizes. This is a quirk: most DHT derivatives cannot aromatize because the structure blocks the aromatase enzyme. Oxymetholone appears to convert to estrogen via a non-standard pathway. This is why users report significant water retention and estrogenic effects that you would not expect from a DHT derivative on paper.

The 17-alpha-alkyl group makes it orally active and hepatotoxic. Hepatotoxicity is rated severe, among the worst in this dataset alongside Superdrol and Halotestin.

Pharmacokinetics and dosing

The dataset records a half-life of about 8 hours, Cmax near 500 ng/dL, Tmax around 3.5 hours, and bioavailability near 95%. The short half-life drives once- or twice-daily oral dosing in documented protocols. Splitting the daily dose smooths the serum curve and tends to reduce peak-to-trough swings in side effects.

Research protocols used oxymetholone in the 25 to 100 mg per day range. Clinical doses for anemia are typically lower. The hepatotoxicity argues for short runs (4 to 6 weeks) and against high doses.

Hepatotoxicity: the defining risk

This is the single biggest harm with Anadrol. Hepatotoxicity is rated severe. Liver enzyme elevation (AST, ALT, GGT) is common, and case reports document cholestatic jaundice and peliosis hepatis (blood-filled cysts in the liver) with oxymetholone use. Some of these cases occurred at therapeutic doses in clinical settings, not just at supraphysiological bodybuilding doses.

Liver monitoring is non-negotiable. Draw AST, ALT, GGT, and bilirubin before starting, mid-run, and after. If enzymes climb sharply, or if you develop jaundice, dark urine, pale stools, or right-upper-quadrant pain, stop immediately and seek medical evaluation. These are signs of serious liver injury, not something to push through.

Duration is as important as dose. A 4-week run at a moderate dose is far safer than an 8-week run at a higher dose. The liver recovers when the stressor is removed, but repeated or prolonged stress raises the risk of lasting damage.

Water retention, blood pressure, and hematocrit

Anadrol drives three cardiovascular-relevant effects simultaneously. First, water and sodium retention from the estrogenic activity increases blood volume and blood pressure. Second, erythropoiesis raises hematocrit, which thickens blood. Third, lipid shifts (HDL suppression) affect vascular health.

The combination is substantial. Blood pressure monitoring throughout the run is essential. If it climbs, the options are dose reduction, stopping, or adding an antihypertensive like telmisartan. Hematocrit should also be monitored, and if it exceeds 52 to 54%, phlebotomy or dose reduction may be needed.

Much of the weight gained on Anadrol is water, not muscle. When the compound clears, the water goes. Understanding this helps set realistic expectations. The scale going up 10 pounds in two weeks does not mean 10 pounds of muscle.

Estrogen management on a DHT derivative that aromatizes

This is genuinely confusing for many users. Anadrol aromatizes, but not through the standard aromatase pathway. This means aromatase inhibitors like Arimidex may be less effective at controlling the estrogenic side effects than they would be on a testosterone cycle.

The practical approach is symptom-driven. If water retention and blood pressure are the issues, a SERM like Nolvadex (tamoxifen) at the breast receptor level may help with gynecomastia symptoms, while dose reduction is the most reliable lever for the water load. Crashing E2 with heavy AI dosing is counterproductive and may not address the non-standard estrogenic pathway anyway.

Detection time

Oxymetholone has a long-term metabolite (18-nor-17β-hydroxymethyl-17α-methyl-5α-androst-13-en-3α-ol) detectable for roughly 21 to 42 days after the last dose (Sobolevsky & Rodchenkov, 2012). Earlier metabolites clear within 5 to 7 days. WADA screens for it.
