Anavar (oxandrolone) is an oral anabolic steroid with one of the mildest side-effect profiles in the oral AAS group. The short answer: it is a DHT derivative that does not aromatize, carries mild rather than severe hepatotoxicity, and has a 9.5-hour half-life that supports twice-daily dosing. It is favored in cutting contexts and in female use because of its high anabolic-to-androgenic ratio. The main real-world concerns are HDL suppression and cost, since real oxandrolone is expensive and widely faked.
What Anavar is and how it works
Anavar is an oral 17-alpha-alkylated DHT derivative. An added oxygen atom at the A-ring (the “ox” in oxandrolone) shifts its activity toward a high anabolic-to-androgenic ratio. It was originally reintroduced clinically for weight gain after trauma, severe burns, chronic infection, and osteoporosis. It still has legitimate medical use in that space.
As a DHT derivative, oxandrolone cannot aromatize to estradiol and is not further 5-alpha-reduced. This removes the estrogenic water retention and DHT-driven androgenic pathways. The 17-alpha-alkyl group lets it survive oral first-pass metabolism, which is what makes it orally active.
The dataset records a half-life of 9.5 hours, Cmax near 772 ng/dL at the 20 mg reference dose, Tmax around 1.6 hours, and bioavailability near 62.5%. That short half-life is why twice-daily dosing is standard in documented protocols, to avoid large swings.
Dosing frequency and pharmacokinetics
The 9.5-hour half-life means once-daily dosing produces a noticeable peak and a low trough by the next dose. Splitting the daily amount into two doses smooths the curve. This is the same logic as splitting injectable esters like testosterone cypionate, just on a faster timescale.
Use the peak-trough estimator to visualize how a once-daily vs twice-daily schedule changes the serum profile. The practical effect of splitting is more stable blood levels, which tends to mean more consistent effects on nitrogen retention and less oscillation in any side effects.
Research protocols have used oxandrolone in the 10 to 50 mg per day range. Clinical doses for weight gain after burns are typically lower, in the single digits to low teens.
Lipid effects: the main harm-reduction target
This is where Anavar gets its reputation for being “mild but not harmless.” Oxandrolone suppresses HDL cholesterol, sometimes significantly, even at modest doses. This is common to oral AAS but particularly noted with oxandrolone in the literature. LDL can rise as well.
A lipid panel before, during, and after any oxandrolone run is non-negotiable for harm reduction. If HDL crashes, that is a cardiovascular risk signal, and the duration and dose matter. Some users add omega-3 supplementation and cardio to counter the lipid shift, but those are partial mitigations, not fixes.
The cardiovascular risk from lipid damage is cumulative. Short, modest runs are less risky than long, high-dose runs. This is a compound where more is not better.
Hepatotoxicity and liver monitoring
Oxandrolone is 17-alpha-alkylated, so it carries liver stress. However, it is rated mild in this dataset relative to other oral AAS like Anadrol or Superdrol, which are rated severe. Clinical use of oxandrolone at therapeutic doses for extended periods (months) has shown it is tolerable for the liver in most patients, but enzyme monitoring still applies.
Draw AST, ALT, and GGT before starting and periodically during. If enzymes climb sharply, the right call is to stop or reduce. The liver recovers when the stressor is removed, but repeated or prolonged stress raises the risk of cholestatic injury.
Female use and virilization risk
Oxandrolone is one of the few AAS used by women because of its high anabolic-to-androgenic ratio. Even so, virilization risk is real: deepening voice, clitoral enlargement, facial hair growth, and hair loss. These effects can be irreversible if not caught early.

Source quality and counterfeiting
Real oxandrolone is expensive to manufacture. This makes it one of the most counterfeited compounds on the market. Common fakes include Winstrol (cheaper), low-dose testosterone, or completely inert fillers. Independent third-party testing (Janoshik, IBR) is the only way to verify what is in the bottle.
This is a major harm. If you think you are taking oxandrolone but are actually taking Winstrol, you get different side effects (joint pain, worse lipids) and different detection windows.

How long does Anavar stay in your system?
The parent compound clears within hours to a couple days, but the long-term metabolite is detectable for up to 18 to 28 days (Guddat 2013). For doping control purposes, plan for about a month.
Does Anavar aromatize or cause gynecomastia?
No. Oxandrolone is a DHT derivative and cannot convert to estrogen. Estrogenic side effects like water retention and gynecomastia are not expected from oxandrolone alone.
