Finasteride

Also called Propecia · Proscar

For lifters worried about thinning hair, finasteride comes up as a way to preserve hair while using TRT or other androgens. The hoped-for benefit is less DHT-related hair loss without giving up training progress. Hair benefits take months; sexual function and mood are important parts of the trade-off.

Hair and training

The hair-versus-gains question

  • Hair takes time

Finasteride treats male-pattern hair loss; benefit generally takes at least three months.

  • What about strength?

An older-men testosterone trial found no loss of measured strength or lean-mass benefits with finasteride.

  • How you feel matters

New sexual-function or mood changes deserve medical attention.

Why it appears in TRT and bodybuilding discussions

The practical aim is to keep a receding hairline or thinning crown from becoming the price of using androgens. Some lifters also ask whether lowering DHT will cost them strength or lean mass. Those are separate questions: a hair-loss treatment is not automatically an anti-muscle treatment.

Will finasteride hurt muscle gains?

A randomized study in men aged 60 and older found that adding finasteride did not alter the measured muscle-strength and fat-free-mass improvements from testosterone. That is useful evidence against the blanket claim that lowering DHT necessarily cancels testosterone’s muscle effects. It does not prove identical results for younger lifters or multi-drug steroid use.

Does it protect hair from every steroid?

That distinction is easy to miss. Finasteride reduces testosterone’s conversion to DHT; it is not a clinical guarantee of protection from every androgen someone might add. Claims that a particular stack is “hair-safe” go beyond the evidence reviewed here.

What do the 1 mg and 5 mg labels mean?

The hair-loss dose and the prostate dose answer different treatment needs. Dose-ranging hair-loss trials found similar efficacy with 1 mg and 5 mg, so the larger prostate dose is not a shortcut to faster regrowth. The visible dose guide keeps the two indications separate.

What is the real trade-off?

The aim is better hair without an unacceptable effect on sexual function or mood. Discuss new symptoms rather than trying to read them from DHT theories or a plot. The EMA advises people taking 1 mg for hair loss to stop and seek medical advice if mood changes occur. Tell the clinician interpreting PSA that finasteride is being used, and keep crushed/broken tablets away from anyone who is or may be pregnant.

The relative curve shows oral drug timing. It does not predict hair density, DHT suppression or the effect of a topical product.

Timing assumptions and sources

The Propecia label gives a 4.5 h first-dose half-life and a 1.3 h last-dose peak time in healthy men. This simplified curve combines those timings as an explicit approximation; it is not a calibrated plasma model.

One 1 mg input peaks at 1 relative unit after a single dose. Other amounts scale proportionally; overlapping doses add when accumulation is on. Different compounds use separate relative scales.

Timing half-life used
4.5 hours
Absorption half-life assumed
0.3151872 hours
Reference amount for the relative scale
1 mg
  • Absorption half-life 0.315187 h is mathematically reconstructed to place the peak at 1.3 h; it was not directly measured.
  • One 1 mg input has a peak of 1 relative unit, independent of the example dose. Linear scaling and repeated-dose addition are illustrative.
  • The source peak 9.2 ng/mL and AUC 53 ng·h/mL are steady-state observations, not single-dose calibration.
  • Saturable enzyme binding and low-dose nonlinearity are omitted; the curve cannot estimate DHT, treatment benefit or a personal stopping interval.

Common questions

Will finasteride stop hair loss while I use TRT?

It is an established male-pattern hair-loss treatment, but its standard hair trials do not guarantee protection for every testosterone exposure or other androgen. The realistic goal is hair preservation assessed over time.

Does finasteride reduce muscle gains?

In one randomized trial in older men with low testosterone, finasteride did not alter the measured strength and lean-mass benefits of testosterone. That supports a narrower answer than either “it kills gains” or “it cannot affect anyone.”

Does more finasteride mean more hair protection?

The cited hair-loss dose-ranging studies found similar efficacy with 1 mg and 5 mg. The 5 mg label treats an enlarged prostate; it is not a proven way to offset an arbitrary steroid dose.

How long until I can judge hair results?

The Propecia label generally requires daily use for at least three months before benefit is observed. The hour-by-hour plot is not a hair-growth forecast.
